Even before Obamacare, less than one quarter of health costs for uninsured persons were paid for out of pocket, regardless of family income. The Obamacare Exchanges have already picked the low-hanging fruit–those sick enough or heavily subsidized enough to make the purchase of coverage worthwhile. Going forward, it will be increasingly difficult to convince the uninsured to pay thousands of dollars to give up a safety net whose actuarial value is 75%.
